Greater Manchester Electronic Long-term Conditions Integrated Review Template ready for testing
The Greater Manchester Electronic Long-term Conditions Integrated Review Template (GM-ELIRT) is currently being reviewed by nurse practitioners and experienced practice nurses prior to recruiting GP practices for testing. The integrated review template has been developed in EMIS PCS by Trish Gray, Malcolm Young and Nurse Practitioner/CLAHRC Secondee Sam Lacey, in collaboration with Health First Ashton, Leigh and Wigan. The template provides an integrated review for patients with multimorbidity and may assist in identifying needs not currently identified by single disease assessments. Clinical advice according to national and international clinical guidelines has been built into the template to guide practice. GPs, practice nurses and nurse practitioners at practices currently using EMIS PCS in Ashton, Leigh and Wigan, Central Manchester and Stockport are being contacted to be the first testing sites. Following initial feedback, the template will be replicated in EMIS Web and others systems, such as Vision and SystmOne, prior to a second wave of testing.
