IGT Care Call short-listed for a HSJ Care Integration Award

The GM CLAHRC is celebrating another outstanding achievement for the Impaired Glucose Tolerance (IGT) Care Call project which has now been shortlisted for Health Service Journal (HSJ) Care Integration Award in the diabetes category.

The Care Integration award, launched by the HSJ and the Nursing times is a new award recognising and rewarding excellence in collaborative clinical practice and runs alongside the Patient Safety awards. The awards are designed to recognise, promote and reward outstanding efforts that demonstrate integration across the different elements of healthcare  and provide flexible, efficient and responsive ervices that meet the need of patients.

Working together the  GM CLAHRC and Salford Diabetes team, developed a six month telephone based, lifestyle intervention programme for people diagnosed with IGT with the aim of preventing them from developing type 2 diabetes. The  programme provided motivation, support and evidence based education via a series of  electronic scripts developed and maintained by the specialiast diabetes team . Working individually with participants, health advisors tailored supporting resources to assist in the achievement of their lifestyle goal which included provision of information about local NHS and community services.

Since the success of the project, the IGT Care Call, which won a Quality in Care award for best type 2 diabetes preventative initiative in 2011, is now being rolled out to all GP practices in Salford and being offered to people diagnosed with impaired fasting glucose (IFG) in addition to IGT.

Professor Ruth Boaden, GM CLAHRC Deputy Director and Lead for Implementation, said: “This is a fantastic achievement which recognises the excellent results we’ve had with the IGT Care Call programme so far. “Providing an intervention to help prevent people from developing  type 2 diabetes saves them from the numerous complications of diabetes later in life. This also means it costs the NHS less in the longer term.

"Patients like the flexible service delivered by phone that motivates and enables them to make positive changes to their lifestyles around diet and exercise.” The next stage of judging takes place  on 17 May when Linda Savas, Knowledge Transfer Associate and Katherine Grady, Care Call Manager, will travel to London to present to a panel of experts detailing the rationale behind the project, how it was implemented, the outcomes and its future development.  The winners will be announced at a ceremony in London on 4 July 2012.
