Integrated long-term conditions assessment tool available to NHS providers and clinical system suppliers
The Greater Manchester Electronic long-term conditions Integrated Assessment Tool (GM-ELIAT) provides an integrated, individualised, holistic assessment involving physical, psychological, social and spiritual needs for patients with multiple long-term conditions. The GM-ELIAT is now available to NHS providers and clinical system companies. It can be used across a multidisciplinary integrated team and is designed to:
- Identify and address unmet needs
- Identify comorbid risk
- Reduce the time spent on the assessment process in comparison to paper-based forms and the use of multiple single disease assessment forms
- Reduce the repetition found when multiple services are involved in an individual patient’s care
- Enhance the quality of assessment by the use of a range of risk assessment tools
- Standardise the assessment process across an integrated health and social care team.
A prototype model was developed and piloted for functionality and feasibility across a community matron service. It received a positive response for a range of items including time efficiency, quality of assessment and for the educational content in the form of embedded evidence-based guidelines. Refinement was ongoing during the pilot to ensure that it remained a usable tool throughout the process. The evaluation report provides further information about the pilot. There are two versions of the GM-ELIAT: Excel 2010 and Excel 2003 with instructions for accessing the content and saving completed copies. A user guide has been prepared to guide potential users through each page, highlighting all of the tool’s functions. A technical guide is available to provide information regarding rules and formatting. The GM-ELIAT can work as a standalone product within a single NHS service; however, for it to perform as a fully integrated tool for sharing the assessment and care planning process, it should be built into a clinical system.
Following positive feedback from community matrons in Trafford during the pilot, Pennine Care NHS Foundation Trust are investigating the feasibility of developing a version of the GM-ELIAT within their clinical system for use by clinical teams across their community services. The Manchester Learning Disability Partnership has also shown interest. CLAHRC GM has worked with clinical system providers such as INPS who are looking to develop a GM-ELIAT module to share with their clients.

NHS organisations can use the Excel versions or build a version of this tool into a clinical system without charge, as long as they acknowledge CLAHRC GM as the originator and retain the copyright statement on the tool. The GM-ELIAT contains a number of third party questionnaires, and therefore cannot be accessed directly from our website. It is the user’s responsibility to gain permission (where required) from the authors/owners for the use of these embedded tools.
