Improving Leg Ulcer Management In community Nursing and Podiatry (ILUMIN)

The ILUMIN project aims to optimise the assessment and treatment of leg management in NHS community services, improving outcomes for patients through the implementation of the following three quality markers:

 

  1. Patients with a leg ulcer receiving an ABPI within 21 calendar days of being seen by the service.
  2. Patients with a venous/mixed aetiology leg ulcer being treated with compression therapy (where suitable).
  3. Patients with a venous/mixed aetiology leg ulcer in full compression, who are being treated with a 2-layer hosiery kit (where suitable).

 

Over the past 15 months the ILUMIN team have been working hard with community nursing teams in:

  • East Lancashire Hospitals Trust (ELHT)
  • Manchester University NHS Foundation Trust (Central MFT, South MFT and North MFT)
  • Salford Royal NHS Foundation Trust (SRFT)

 

The teams have been working together to implement the ILUMIN plan through a co-design approach, to ensure that the project operates optimally in each location.

 

ELHT update

Data collection recently completed in East Lancashire. A final workshop was led by Aneela and Louise to share initial data finding and to discuss how teams were going to carry on with the improvements now the project was complete. Interesting discussions were had and it was really good to hear that the project had got the nursing team talking more about wound care management - they really felt the culture had been changed within the teams.

Central MFT (CMFT) update

Data collection has been ongoing at CMFT for the past 12 months. Aneela and Louise are keeping in close contact with each of the different area teams to support the nurses with ILUMIN and ensure that there is a steady supply of data collection to aid improvements.

 

South MFT (SMFT) update

The ILUMIN team ran their first workshop in SMFT in early October this year. The wound care nurses were really enthusiastic about the project and data collection has begun, and will run through to summer next year.

North MFT (NMFT) update

NMFT are the latest group to come on board. ILUMIN was launched at the end of October and data collection has now begun. Regular workshops are scheduled to promote ILUMIN and help troubleshoot any issues the clinical team may be having with data collection.

SRFT update

Alison, Aneela and Louise have been meeting with the SRFT clinical governance team to understand how best to implement ILUMIN with the Trust. Discussions about optimising the Electronic Patient Record (EPR) to collect information on the three quality markers is currently ongoing.
