NIHR CLAHRC GM to Evaluate Impact of Local Long Term Conditions Early Intervention Test Bed as Part of National NHS Programme

Heywood, Middleton and Rochdale Clinical Commissioning Group (HMR CCG) has appointed NIHR CLAHRC Greater Manchester to carry out an evaluation of the implementation and effect of its Long Term Conditions Early Intervention Programme, which aims to improve care for people who have, or may develop, certain long term health conditions by anticipating their health needs before they happen.

 

The project is part of NHS England’s national test beds programme which aims to develop and test new applications of technology in the NHS in order to provide better care for patients as well as better value for taxpayers.  HMR CCG’s test bed project combines new approaches to data analysis with the application of telehealth in order to help identify and support patients at risk of hospital admissions due to specific long-term conditions. Three conditions have been identified as the project’s initial areas of focus - Chronic Obstructive Pulmonary Disease (COPD), heart failure and Type 2 diabetes –all of which are considered to be the areas where the test bed could make the biggest difference.

 

NIHR CLAHRC Greater Manchester will work closely with HMR CCG and the partners providing the various elements of the intervention ( MSD and Verily), which is being deployed in general practices across Heywood, Middleton and Rochdale, in order to evaluate its implementation and its effect on several health outcomes.

 

NIHR CLAHRC GM’s evaluation will involve two areas of focus - process evaluation and service evaluation.  The process evaluation will explore the acceptability of the intervention to health care professionals and how their behaviour fits with that intended by the programme leaders, whilst the service evaluation will use hospital emergency department data to investigate any difference in health service utilisation pre and post-implementation within the HMR CCG footprint, as well as in comparison with another GM CCG.
