Tool launches to improve the management of heart failure in primary care
The Greater Manchester Heart Failure Investigation Tool (GM-HFIT) is now available for use in primary care. GM-HFIT verification level 1 guide and template The level 1 GM-HFIT verification tool and ‘how to’ guide provide step-by-step guidance to conduct an assessment of heart failure registers. The tool looks at diagnosis, investigations, medical therapy and validation of appropriateness to be on the register, and aims to ensure both that heart failure registers are accurate and that evidence-based management of heart failure is promoted. GM-HFIT verification level 2 guide and template The level 2 verification tool and guide provide information on how to conduct a more in-depth review of general practice heart failure registers to determine the accuracy of registers as well as the quality of management. There are two elements to this process: validation of the register and a skills audit to ensure patients are managed appropriately. The skills audit is comprised of 12 indicators based on National Institute for Health and Care Excellence (NICE) and European Society of Cardiology heart failure guidance. The tool aims to ensure that heart failure registers are accurate, promote evidence-based management of heart failure and provide practices with a dashboard that identifies of areas of heart failure management that require improvement. GM-HFIT case-finding guide and template The GM-HFIT case-finding guide and template support practitioners in the identification of individuals who have heart failure but are not on the practice register. The guide and template aim to identify people with heart failure not currently recorded on the register, promote evidence-based treatment and effective monitoring of all patients diagnosed with heart failure and increase the reported prevalence of heart failure.
