The Greater Manchester Electronic Long-term Conditions Integrated Assessment Tool (GM-ELIAT) is currently being tested by the Trafford Community Matron Service. The tool aims to provide an efficient method for sharing an assessment and management process across multidisciplinary health and social care teams. It has been designed to identify and address unmet needs, identify co-morbidity risk, and structure and standardise the assessment process for patients with multimorbidity. Formulas built into the tool, based on the results of embedded health questionnaires, demographic details and inputted data, will assist the healthcare assessor in making clinical and referral decisions according to current research evidence and national and international clinical guidelines. The tool has been developed by the Project Lead, Trish Gray and Data Analysts, Caroline O’Donnell and Malcolm Young. The design was informed by a literature review, patient interviews and discussions with health and social care professionals and clinical specialists, to develop a greater understanding of multimorbidity. Community Matrons are completing a series of Plan, Do, Study, Act (PDSA) cycles to compare the tool to their current assessment process. If successful, other community matron/active case management services will be recruited prior to extending the testing remit to other community services such as rehabilitation or social care.
